1. It's Too Small

By far the most common cause. A QR code that scans perfectly on a screen can fail once printed small - on a business card, a product label, or a shrunk-down flyer. As a rough guide, a QR code needs to be at least 2 x 2 cm for reliable scanning at arm's length, and considerably larger if it'll be viewed from further away.

2. Poor Contrast or a Busy Background

QR scanners need clear contrast between the code and its background to detect the pattern. A code placed over a photo, a gradient, or a similar-toned background can become unreadable even though it looks fine to a human eye.

Fix: Use a plain, high-contrast background - dark code on white is the safest choice. If design constraints require a busy background, add a solid white or light box behind the code itself.

3. No Quiet Zone Around the Code

QR codes need a margin of empty space around their edges - called the quiet zone - for scanners to correctly identify where the code starts and ends. Cropping too tightly, or placing text or graphics right up against the code, breaks this.

Fix: Leave at least a small margin of empty space on all sides, roughly the width of one of the code's own squares, and don't let other design elements crowd right up to the edge.

4. Physical Damage or Poor Print Quality

Creases, smudges, low print resolution, or a code printed on a curved or reflective surface can all interfere with scanning, even when the original file was perfectly fine.

Fix: Check the physical print itself - is it crisp, undamaged, and flat? Matte materials generally scan more reliably than glossy or reflective ones, which can create glare that confuses a camera.

5. Poor Lighting or Glare

Sometimes the code itself is fine, but the scanning conditions aren't - a dim room, harsh glare from an overhead light, or a reflective laminate over the print can all prevent a successful scan.

Fix: If you control the environment (like a physical shop display), consider lighting and placement. If you don't, this is a good reason to be generous with code size - a larger code is more forgiving of imperfect scanning conditions.

6. The Content Itself Is the Problem

Sometimes the code scans successfully, but nothing happens afterward - which looks identical to a scan failure from the user's point of view. This usually means the encoded content itself has an issue:

  • A URL with a typo, or one that's since gone offline

Fix: Test the encoded link directly in a browser, separately from scanning the code, to confirm the destination itself actually works.

Most modern phones scan QR codes natively through the camera app, but if someone is using a dedicated third-party QR scanner app instead, older or poorly-maintained apps can have genuine bugs or compatibility issues.

Fix: If a specific person reports trouble, ask them to try scanning with their phone's built-in camera app rather than a separate scanner app, if they aren't already.
